How to set up a MyChart account.

Create and access your MyChart account

Patients 13 years and older can create a MyChart account.

Request a proxy account

Parents and legal guardians can request proxy access to their child or teen's record by signing into their own MyChart account and going to the Friends and Family Access page to submit a Proxy Access Request form. (Note: You will need to sign in to your MyChart account to access this page).

If you have an activation code from your doctor

  1. Look for an activation code on your After Visit Summary or in an email or text message from your healthcare organization.
  2. Follow the instructions provided with your activation code to create your MyChart account.
  3. Go to MyChart
  4. Click on sign up with activation code.
  5. Enter your activation code, date of birth, and phone number.
  6. Update your contact information, including your mobile number and email address.
  7. Set up two-step verification to confirm your identity.
  8. Create your username and password.
  9. Review and accept the terms and conditions.
