What is a Medical Home?

A medical home is a patient-centered model for delivering healthcare that provides patients with comprehensive, high-quality and cost-effective care. The medical home goal is to deliver primary care that is accessible, continuous, family-centered, coordinated, compassionate and culturally appropriate.

Expanded access – We continue to care for you, even when your clinic is closed.  Use eCare to send a message to your care team, or call the clinic anytime during the day or night to receive prompt medical advice or instructions from one of our medically trained staff members. 

Electronic medical records – Our providers have your entire UW Medicine health record at their fingertips.  Our ability to review your record allows us to provide you with comprehensive and safe care. 

Your care meets your needs – Developing a relationship with your healthcare team is critical to providing you with care that meets your personal, cultural and healthcare needs. 

We are here to support you – Our healthcare team works with you and your family to help you manage and reach your healthcare goals. 

Coordinated care – Our healthcare team coordinates your care with specialists, hospitals and/or other providers to ensure that you receive the best care possible. 

Your feedback matters –As a UW Neighborhood Clinic patient, you may receive a satisfaction survey in the mail.  Whether you identify something we are doing well or something we may need to improve, our leadership and care teams carefully review and follow-up on your feedback. 

Electronic access to your health information – With UW Medicine eCare​, you have secure, online access to your personal health information. As a registered eCare user you can email your healthcare team, schedule and cancel appointments, order medication refills and review your medical history and lab results.
