How to Refer a Patient
To speak to someone in person about a referral, call the UW Medicine Contact Center at 1-877-520-5000 or 206-520-5000.
For online referrals, use the Provider Referral Form.
To refer by fax, print out and send the Referral Request (PDF) to the clinic fax number. To find the clinic fax number, search for the clinic from our Search Locations page.
To refer a patient to a specific cancer clinic, please consult our cancer referral contact list.
To refer a patient to cardiology or cardiac surgery, please consult the Regional Heart Center Referrals page.
To refer a patient to a specific transplant clinic, please consult our transplant referrals contact list.
Access to medical records for referring physicians
U-Link is a secure web portal that allows referring physicians, other licensed referring healthcare practitioners and their support staff to access the electronic medical records of the patients they refer to us. U-Link provides access to the records of patients at UW Medical Center (UWMC), Harborview Medical Center (HMC) and Seattle Cancer Care Alliance (SCCA).
U-Link allows providers to view the same records to which UW Medicine medical providers and medical staff have access, including:
- Lab results
- Dictated notes and transcripts
- Radiology reports and images
- Pathology reports
- Medication lists
Referring providers may print or copy portions of the record as needed. U-Link is offered free of charge.
Around-the-clock medical consultation
Medcon is a toll-free consultation and referral service of the UW School of Medicine and its academic medical centers, Harborview Medical Center and UW Medical Center.
As a major resource for education, medical discovery and patient care, UW School of Medicine places particular importance on its communication with practicing physicians. The line is open 24 hours a day, seven days a week. A Medcon consultation coordinator will connect you with the appropriate expert.
