How to Connect Apple Health or Google Fit to Track My Health
Welcome to the UW Medicine Track My Health program! This packet includes information to help you connect your Apple Health or Google Fit smartphone application to Track My Health. You can always reach out to your care team with any other questions you have.
What is Track My Health?
Track My Health allows your health data to be collected into your MyChart remotely as part of your care plan. Your healthcare provider can then monitor your health remotely and remain informed about your condition between visits.
You can provide your health data automatically via a smartphone application or manually into Track My Health. If you choose to have the information entered automatically, Apple Health (iPhone) and Google Fit (Android) devices are required to connect to Track My Health.
How to Connect Track My Health to Apple Health or Google Fit
If your healthcare provider has placed an order to track your health remotely and you have an active MyChart account, you are ready to synchronize your mobile device to MyChart.
If you have not signed up for MyChart, you will need to sign up at How to Manage Your Healthcare with MyChart | UW Medicine
- Click View Message from the message notification on the MyChart Homepage.
- Click embedded hyperlink to access the Track My Health activity.
- Link smartphone applications to MyChart:
- iPhone (Apple Health) users will click Connect to Health to link the MyChart app to the device account.
- Android (Google Fit) users will click Connect Google Fit to link the MyChart app to the device account.
- You will see available data begin to synchronize.
iPhone:
Control data from device settings:
- From the device Settings > Health > Data Access & Devices > MyChart.
- Choose to Turn Off All or toggle individual data points.
How to Manually Enter Data to “Track My Health”
- Choose Track My Health from the main menu.
- Click an active flowsheet you are entering data for.
- Click Add new data.
- Enter date, time, value(s) and click Continue
- To enter additional data values, click Add another reading.
- Review data and click Submit
