The Web Intake Request (WIR) form is not supported by Internet Explorer. Microsoft stopped active development of Internet Explorer in January 2016. If you would like to use the WIR and you currently use Internet Explorer, please find a more modern alternative like Google Chrome or Mozilla Firefox. If you are required to use a Microsoft browser, you can submit your request through email at uwmweb@uw.edu. NOTE: Email requests will take more time to process and complete. Using the WIR form is recommended.
