UW Medical Center Credentialing

This credentialing process ensures that non-physician individuals who interact with patients at the University of Washington Medical Center are competent to practice in their role and have current immunizations to ensure the safety of the patients.

DETERMINING WHETHER YOU NEED TO CREDENTIAL WITH PATIENT CARE SERVICES (PCS)

What is your role at UW Medical Center?

Physician, Nurse Practitioner, or Physician Assistant

You do NOT need to credential with PCS. Please contact the Office of Medical Staff Appointments known as OMSA at omsa@uw.edu

Teaching

If you are NOT a Physician or Nurse Practitioner AND if you will be supervising students in the clinical setting, you do NOT need to credential with PCS. Please contact the Staff Development Specialist at lgo@uw.edu

Research or Clinical Care Non-Physician

If you are NOT a Physician, Nurse Practitioner, Physician Assistant or Clinical Faculty AND you will be conducting research or clinical care with UWMC patients, you DO need to credential with PCS.

PCS Credentialing Attestation Application

Your employer falls into one of these two categories:

UW/UW entity personnel

(e.g., HMC, Valley Medical Center, Airlift NW, UW School of Medicine departments, UW School of Pharmacy, UW School of Public Health)

Application

  1. Fill out the UWMC Attestation Application
     
  2. Scan, save as PDF and email the signed application and proof of your COVID-19 vaccination to pcscred@uw.edu

    Incomplete applications will NOT be accepted. Please allow at least 2 weeks for processing.

    Confirmation of approval will be sent via email. Upon approval you will wear your employer-issued photo ID badge at all times in the Medical Center.

Non-UW/UW entity personnel

(e.g., Bloodworks NW, Kaiser, VA Puget Sound Health Care System, Fred Hutch Cancer Center and Seattle Children’s Hospital)

Application

  1. Fill out the UWMC Attestation Application

    Your application will include a background check by UWMC Public Safety. Please fill out the Criminal Background Authorization Form, and include it with your application.
     
  2. Scan, save as PDF and email ONLY the signed application and 2 background check forms to pcscred@uw.edu

    Incomplete applications will NOT be accepted. Please allow at least 2 weeks for processing.

    Confirmation of approval and authorization for a UWMC photo ID badge will be sent via email. You will wear your UWMC photo ID badge at all times while in the Medical Center.

After Approval

  • An electronic copy of your approved Attestation Application will be retained by Patient Care Services.
  • You are responsible for maintaining and keeping current the records in your employee file including:
    • Immunizations updated per UWMC Employee Health schedules
    • Annually updated confidentiality form
    • Current licensure with the State of Washington (if applicable to role at UWMC).
  • You will be required to provide a copy of your file within 24 hours of our call or email for auditing purposes.
