Health Information Exchanges and Care Everywhere

UW Medicine participates in electronic sharing of medical records through health information exchanges such as Care Everywhere. Care Everywhere is a way for providers in different healthcare systems who are caring for the same patient to securely and efficiently share medical information through our electronic health record system for treatment purposes. All UW Medicine patients benefit from Care Everywhere unless you choose to opt out. This sharing of healthcare information ensures that critical health data is made securely and promptly available to other providers involved in your healthcare who participate in Care Everywhere at no cost.

What information is shared through Care Everywhere?

Information shared through Care Everywhere may include:

  • Allergies
  • Clinic and hospital visits
  • Diagnoses
  • Immunizations
  • Medications
  • Test results

What is shared is standard across all patients and is overseen by a UW Medicine governance group.

Who can view my information shared through Care Everywhere?

Healthcare organizations who are part of the Care Everywhere network and are involved in your healthcare can access your information through Care Everywhere. 
You can find other providers who participate in Care Everywhere on the Care Everywhere Epic page.

Insurance providers do not have access to Care Everywhere.  Care Everywhere sharing is standardized across participating providers, making information available to any participating provider involved in the patient’s care, so vital records are accessible to future caregivers who are not yet known.

Care Everywhere is different than MyChart, which is a patient portal to access your healthcare information. Learn more about MyChart on the Manage Your Healthcare with MyChart page.

How does Care Everywhere benefit me?

Participation in Care Everywhere gives providers outside of UW Medicine quick access to important clinical information to ensure that they have a more complete set of information about your health when providing care. It also gives UW Medicine providers the same benefit by providing access to your health information from other healthcare organizations, saving time and money. Without Care Everywhere, providers must request records and wait for them to arrive, often faxed, or in some other less secure format. This requires resources on both sides and takes longer for the information to get into the hands of your provider.

What if I want to opt out of Care Everywhere?

You may choose to opt out of Care Everywhere by filling out this form “UW Medicine Epic Care Everywhere Patient Opt-Out.” Opting out of Care Everywhere will not affect your access to healthcare services at UW Medicine. Opting out will prevent providers from viewing your medical records through Care Everywhere in situations that may affect your care, in particular when providers may benefit from quick access to your medical records.

If you opt out of Care Everywhere, UW Medicine will continue to share your medical records by fax, mail or email if a provider requests access to your medical records to treat you in accordance with applicable laws and regulations.
