If you’ve picked an “Office visit” we will ask you to choose what location you want to make the appointment for. This page will automatically show you most available locations or you can use the search box in the right upper corner above the locations listed to see if another clinic is in your area. Enter your zip code in the box and either click the arrow or hit the enter key.
