How Do I Get Started?

Thank you for your interest in becoming a UWMC Volunteer! To be considered for placement in the UWMC Volunteer Program you must complete the volunteer application (please note that applying to the program does not guarantee placement).  All items must be turned in together, including the Reference Form.  All required immunizations must be completed at the time of application.  Once the application has been reviewed and accepted you will be contacted to schedule an interview.

Please print the application, complete it, and return it to the UWMC Volunteer Services office. The application can be dropped off in person (between 8:00am-4:30pm Monday-Friday) or mailed:
