Getting Started with your U-Link Enrollment

Thank for submitting your enrollment form to establish your U-Link account with UW Medicine which provides access to your patients’ electronic medical records. Establishing a U-Link account requires two initial steps:

  • Identify Verification
  • Obtaining Your UW NetID account

Your commitment in following through with each and every step is critical to ensuring that the highest standards of privacy are upheld for your patient. Successful completion of each of these steps will allow you and your staff to have an efficient way to strengthen continuity of care between the healthcare settings for the person we care about most—your patient.

This webpage precedes the first step: Identity Verification. When you click on the link below, you will be taken to an online form that will lead you through the process of submitting the information required to confirm your identity. You will have two options to verify your identity:

  • Fax a copy of your photo ID along with a registration form that you will print out. A letter will be mailed to the address on your photo ID (usually your home address) with a specific URL and a confidential code to obtain your UW NetID online. This takes about a week.

--or--

  • Go to a notary public to confirm your identity on a registration form that you will print out, which you then fax. An email will be sent to you with a specific URL and a confidential code for you to obtain your UW NetID online. This takes about 2-3 business days

After you complete this step, you will be able to create your UW NetID and password, which will then allow you to access your U-Link account.
