Appointments and Referrals
Patients on managed care plans usually must be referred by a primary care provider. Self-referrals are acceptable in certain circumstances.
Providers making a referral should complete a UW Medicine Referral Request (PDF). Questions about referrals or about the Stroke Clinic and its services should be directed to the nurse and/or scheduling coordinator.

Why do our communities need TeleStroke?
With TeleStroke, consulting partners receive immediate telephone consultation with a vascular neurologist. The specialist can review the patient’s digital radiology images via a virtual private network and provide immediate feedback to the physician requesting consultation. Referring partners also have access to acute stroke evaluation at UW Medicine, as well as diagnostic and treatment protocols to facilitate each patient’s treatment.
As part of in-depth training provided with the Telestroke Partnership Program, you can learn more about giving tPA (tissue plasminogen activator) and the indications for other acute interventions for ischemic stroke patients, including interventional neuroradiology techniques. If you are unable to perform this treatment, and if the timeframe is favorable, we can initiate an emergent transfer to Harborview Medical Center for reperfusion treatment.
Individualized approach to care
Thanks to the Telestroke Partnership Program’s individualized approach, our vascular neurologists, neurosurgeons and interventional radiologists can help you determine whether your patients will be best served by staying at your hospital or being transferred to the UW Medicine Stroke Center at Harborview.
Access to UW Medicine stroke experts provides a safety net for you and your hospital. Our goal is to help you care for your patient at your facility. However, UW Medicine health-care providers at Harborview can provide the full continuum of care if the patient warrants a higher level of treatment.
Access to the Transfer Center
The UW Medicine Transfer Center
is staffed 24/7 by experienced, specialized registered nurses who assist referring providers with all aspects of patients’ transfers to UW Medicine’s world-class academic health system. This system includes Harborview, UW Medical Center
and Northwest Hospital & Medical Center
. The transfer center partners with Airlift Northwest
to provide air medical transport.
If you transfer your patient to Harborview, you will remain connected to your patient’s care through U-Link
, a service that allows access to a patient’s electronic medical records. Providers can print or copy portions of the record as needed.
