**BLS Certification MUST be valid through March 2019**

Please complete the training between March 1st and May 31st  2017. 
You will be exempt from BLS training at your site's Orientation/Immersion Program in August.

2017-18 Immunizations Forms: there are 2 forms to be completed by your healthcare provider:

1.  Immunizations Form

2.  TB Testing Form

These forms will need to be completed by your healthcare provider and sent, along with supporting documentation, to the Health Sciences Immunizations Program (HSIP) at

Immunizations compliance for medical students is managed by the Health Sciences Immunizations Program (HSIP). For information about immunizations requirements for Health Sciences students, please visit the Health Sciences Immunization Program (HSIP) website.

A 2-Step PPD is required. This means two PPDs (tuberculin skin tests) must be placed within 1-3 weeks of each other. At a minimum it takes 10 days to receive the results of the 2 step PPD and 4 visits to a provider to have your PPDs placed and read.

If you are PPD-positive please refer to the HSIP website for instructions. Please note, you will also receive a PPD skin test in March 2018. This will place all students due for their annual PPD at the same time each year. The SOM conducts annual TB testing for all students in March every year. If you receive your second PPD earlier than March 1, 2017, you will be off-cycle in March 2018.

If your provider signs off on your immunizations form prior to completing all vaccine requirements, you will need to email subsequent documentation to HSIP in pdf format at  If your Hepatitis B or Varicella vaccine series are ‘in process’, you will be considered compliant as long as you adhere to your vaccine schedule.​

After emailing your Immunizations Form to HSIP, you will receive a response from HSIP about your compliance status. If your status is non-compliant, make sure to follow their instructions. Immunizations compliance is determined by HSIP, not by the SOM. HSIP will send us weekly reports on student compliance status.

The SOM Compliance Office does not process or interpret student health records and we cannot advise students about the requirements. For questions about immunizations, contact HSIP directly via email at​ or by phone 206-616-9074.
