Accepted applicants are required to initiate the criminal background check (CBC) administered through the
American Medical College Application Service (AMCAS). Washington state law requires that all individuals who have access to children under 16 years of age, developmentally disabled people, and other vulnerable persons, disclose background information concerning crimes and offenses against these populations. A nationwide criminal background search is mandatory for all accepted applicants. The accepted student must initiate the search within two weeks of receiving the acceptance notification e-mail.
Criminal Background Check
Upon acceptance, you will receive an email notification from Certiphi Screening Inc., requesting that you log in to their website and complete the criminal background check. If you have already completed the CBC for other schools you have been accepted to, the report will automatically be released to the UW SOM (please note this only applies to schools that are participating in AMCAS).
Request for Criminal History Information, Self-Disclosure, Consent, and Release of Information form
Certiphi will then send you a second email requesting that you complete the
Request for Criminal History Information, Self-Disclosure, Consent, and Release of Information form. Please print out the form, fill it out, sign it and return it directly to the UW SOM Office of Academic Affairs. Contact information is on the form. If you did not receive the email from Certiphi, you can download the form by clicking on the link above.
Non-Washington Residents - Washington State Patrol CBC
All incoming students are required by law to complete a Washington State Patrol background check. If you are not a Washington State resident, Certiphi will send you a third email requesting that you log back into their website and complete this additional cbc. You will be asked to authorize a $50 charge to your credit card. If you only have one name and no aliases, the actual charge will be $14. The balance will be refunded to your card upon completion of the cbc.
You will be contacted by our office prior to Orientation if we have not received any of the above items.
