How to Make a Referral for Oncology Services

Making a Referral

Physicians may contact individual clinics for direct referrals or call MedCon, the UW School of Medicine's 24-hour consulting physician line at 1-800-326-5300.

Oncology care is available at UW Medical Center and the Seattle Cancer Care Alliance. The Seattle Cancer Care Alliance represents the University of Washington Academic Medical Center's partnership with the Fred Hutchinson Cancer Research Center and Children's Hospital & Regional Medical Center for extended multi-disciplinary consultations and care.

Breast Care and Cancer Research Program

(Seattle Cancer Care Alliance)
The BCCRP holds multidisciplinary clinics on Tuesday and Friday afternoons at the Seattle Cancer Care Alliance. The clinics are staffed with medical oncologists, radiation oncologists, and surgical oncologists who review patient records with breast cancer specialists from pathology and radiology mammography. Breast cancer patients can be referred to the patient care coordinator at 206-288-2166.

Gynecology Oncology Clinic 

(Seattle Cancer Care Alliance)
A combined radiation and gynecology oncology clinic meets daily at the Seattle Cancer Care Alliance. Radiation oncologists and gynecologists who specialize in the care of women with cancer of the reproductive organs provide comprehensive consultations, recommendations and treatment options that may include surgery, radiation, chemotherapy, and/or combinations of those modalities. Patients may be referred to the Gynecologic Oncology patient care coordinator at 206-288-2273.

Hematology Malignancies Clinic 

(Seattle Cancer Care Alliance)
The Hematology Malignancies Clinic meets once a week on Thursday afternoons at the Seattle Cancer Care Alliance. This service provides consultation, recommendation, and comprehensive treatment options for patients with blood disorders.
Referrals should be directed to the patient care coordinator at 206-288-1260.

Infusion Treatment Center

(Seattle Cancer Care Alliance and UW Medical Center, 8-SE)
Our outpatient chemotherapy and infusion centers are located at the Seattle Cancer Care Alliance and on the 8SE floor of UW Medical Center; both locations are equipped to accommodate outpatient procedures such as catheter draws, injections and infusions and are staffed by highly trained and experienced nurses. Recliner chairs and beds are available to outpatients Mondays through Fridays, 8 a.m. to 5 p.m., with after hours, emergency and holiday care available at both locations. Patients are referred to the infusion center following an evaluation by an oncologist from one of the specialty clinics.

Adjuvant Chemo-Radiation Therapy
Some cancer patients benefit from concurrent therapies of chemotherapy and radiation given simultaneously. Concurrent therapy is used primarily to treat gynecologic tumors, gastrointestinal tumors, and some lung and head and neck tumors. Chemotherapy can be coordinated for outpatients in the infusion center, as part of an inpatient regime, or by using a home infusion pump in conjunction with the radiation schedule.

High-Dose Chemotherapy and Bone Marrow and Stem Cell Transplant Service
Our bone marrow and stem cell transplant service is coordinated with the Medical Oncology outpatient service. Arrangements will be made for patient evaluation in a Medical Oncology clinic, located on 8SE. Providers are welcome to make direct referrals to individual specialists. New patient consultations are arranged by patient care coordinators at 206-288-1024.

Medical Oncology Clinic

(Seattle Cancer Care Alliance)
Medical Oncology clinics meet Mondays through Fridays, 8 a.m. - 5 p.m. at the Seattle Cancer Care Alliance. Patients are directed to individual oncologists who specialize in specific diagnoses or disease sites. New patient consultations are arranged by patient care coordinators. The operator will direct your call to the appropriate coordinator. Direct referrals are welcomed by individual specialists. Call 206-288-1024.


Melanoma Clinic

(Seattle Cancer Care Alliance)
The Melanoma Clinic meets each Tuesday at the Seattle Cancer Care Alliance. This multidisciplinary clinic is a collaborative effort by surgical oncologists and medical oncologists to meet the specific needs of patients with a diagnosis of melanoma. Treatment options include node mapping, surgery and/or protocol-driven chemotherapy. Referrals should be directed to the patient care coordinator at 206-221-7404.


Neuro Oncology Clinic

(UW Medical Center's Cancer Center - 1st Floor)
The Neuro Oncology Clinic is a multidisciplinary clinic that meets Wednesday afternoons and Thursday mornings in the Cancer Center, 1st floor. Neurosurgeons, neuro oncologists, and radiation oncologists who specialize in the treatment of brain tumors and disorders combine their knowledge and experience to provide patients with evaluations, recommendations and extensive treatment options. Referrals may be directed to the Neuro Oncology patient care coordinator at 206-543-3572, or the Radiation Oncology patient care coordinator at 206-598-1905.

Pain Clinic

(Seattle Cancer Care Alliance and UW Medical Center's Cancer Center – 1st Fl.)
Our Pain Clinics meet on Tuesday mornings at the Cancer Center and afternoons at the SCCA. An anesthesiologist, who specializes in cancer diagnoses, provides UW patients with evaluations, recommendations, and treatment options. Referrals may be directed to the patient care coordinator at 206-598-1905.


Prostate Brachytherapy Clinic

(UW Medical Center's Cancer Center – 1st Floor)
The Prostate Brachytherapy Clinic meets on Tuesday afternoons and Wednesday mornings. Radiation oncologists and urologists specializing in the diagnosis and treatment of prostate cancer combine their expertise and experience to provide evaluations, recommendations and treatment options. Treatment recommendations may include hormonal therapy, surgery, radiation therapy, seed implant, or combinations of those modalities. Referrals may be directed to the patient care coordinator at 206-598-1905.


Radiation Oncology Clinics

(UW Medical Center's Cancer Center – 1st Floor)
Radiation Oncology clinics meet Mondays through Fridays, 8:00 a.m. - 5:00 p.m. Patients are generally directed to radiation oncologists who specialize in specific diagnoses or disease sites. More information is available at www.uwcancercenter.org. New patient consultations are arranged by patient care coordinators. Cancer Center operators will direct your call to the appropriate coordinator. Direct referrals are always welcomed by individual specialists.
Call 206-598-4100. *For after-hours emergent referrals, please call 206-598-6190 (paging operator) and have the radiation oncology resident on call paged.

Radiation Treatment Center

(UW Medical Center's Cancer Center – 1st Floor)
The Radiation Treatment Center is available to patients 7 a.m.-7 p.m. Mondays through Fridays, with inpatient and emergency care available on holidays, after-hours, and weekends. Physicians, therapists, nurses, patient service representatives, physicists, a dietician, and a social worker combine efforts to provide total patient safety and support during the course of treatment. Patients are referred to the radiation treatment center following an evaluation by an oncologist in the Radiation Oncology clinic.

Sarcoma Clinic

(Seattle Cancer Care Alliance)
The Sarcoma Clinic is a multidisciplinary clinic that meets on Tuesday mornings. Cases are presented at the multidisciplinary sarcoma conference where cancer specialists from the University of Washington's Bone and Joint Center and radiation oncologists who specialize in the treatment of patients with a diagnosis of sarcoma combine their experience and knowledge to provide recommendations and unique treatment options. Referrals may be directed to the Sarcoma Clinic patient care coordinator at 206-288-2167.

Medical Records

The review of outside records is a critical part of a patient's overall evaluation. Patient care coordinators will assist referring physicians to coordinate the following outside records:

  • Copies from the patient's medical record should include information such as operative reports, discharge summaries from hospitalizations, treatment summaries, lab results, history and physical, clinic notes, etc. Copies of medical records will become a part of the patient's UW Medical Cetner chart.

Pathology slides and reports of applicable biopsies and/or surgeries.

Pathology slides that are reviewed at UW Medical Center or the Seattle Cancer Care Alliance will automatically be returned by the Pathology Department.

Radiology films and reports (X-rays, bone scans, CT scans, MRI's, mammograms or any other diagnostic films that may relate to the diagnosis). Films that are reviewed at UWMC will automatically be returned by the Department of Radiology. Copies of films may become part of the patient's UW Medical Center radiology file.

Patient Referrals and Insurance Reminders

If your patient belongs to an HMO or managed care plan, a referral form from the patient's primary care physician should be mailed or faxed to the patient care coordinator who is coordinating the appointment.

Patients will be asked to bring their insurance cards. If a patient's insurance requires pre-authorization, a confirmation should be hand-carried at the time of appointment or mailed or faxed to the appropriate patient care coordinator before the appointment. If the patient's insurance requires a copay, it will be collected at the time of the appointment.

Appointment Confirmation

Patients who are new to UW Medical Center or the Seattle Cancer Care Alliance will be fully registered through the patient care coordinator before the appointment date and can then go directly to their appointment location for check-in.

A confirmation of the patient's appointment date, time, and physician will be faxed to the referring provider. If time allows, a confirmation of appointment will be mailed to patients along with a new patient packet of information, including directions to UW Medical Center or the Seattle Cancer Care Alliance. Directions and a map may also be viewed online.

Follow-Up

The physicians at the UW Medical Center's Cancer Center and the Seattle Cancer Care Alliance are committed to keep referring providers as active participants in their patient's care. After your patient's initial appointment at the Cancer Center, we will send a copy of the evaluation and recommendation. For patients receiving ongoing care or treatment at UW Medical Center or the Seattle Cancer Care Alliance, we will continue to send copies of clinic notes, treatment summaries, etc. Our goal is to provide the specialized care you are requesting.

For Your Patient

As soon as your patient's first appointment is scheduled, we will send them our "New Patient" packet, which includes directions to UW Medical Center or the Seattle Cancer Care Alliance, depending on the appointment location.
